Important items required at sign-up:

¨ Parent or legal guardian with checkbook, credit card or cash

¨ Copy of birth certificate or passport

¨ 2 x 3 current photo (no hats or sunglasses)

¨ 2- 8 ½ x 11 copies of 2009/2010 report card showing all 4 quarters or 3 trimesters (when available)

¨ A physical dated after January 1, 2010 is required.  Form will be provided
